Mental Health Records & Psychotherapy Notes
Because Purity Wellness Clinic provides mental-health services, we recognize the highly personal and confidential nature of therapy records.
HIPAA provides additional protections for psychotherapy notes when those notes meet HIPAA’s definition and are maintained separately from the remainder of the clinical record.
Psychotherapy notes are different from ordinary clinical documentation. SimplePractice & Our Client Portal
Purity Wellness Clinic uses SimplePractice as an electronic health record and practice-management platform.
Clinical documentation and other client information may be maintained through SimplePractice, including, as applicable:
- Clinical records
- Psychotherapy notes
- Treatment plans
- Intake documentation
- Client forms
- Appointment information
- Billing information
- Secure client communications
- Telehealth services
- Other information related to your care
When you access the SimplePractice Client Portal or another designated secure system, additional privacy and security practices associated with that platform may apply.
We encourage clients to use our designated secure systems when communicating sensitive clinical information rather than ordinary email or general website forms.
Google Analytics & Cookies
Purity Wellness Clinic uses Google Analytics to help us understand how visitors interact with our public website and to improve website performance and user experience.

Submitting a website inquiry does not by itself establish a therapist-client relationship with Purity Wellness Clinic or one of its clinicians.

When sensitive information is necessary for treatment, clients should use the secure communication or client portal methods provided by Purity Wellness Clinic whenever available.

Existing clients should use the secure communication methods provided by their clinician or through our designated client platform when appropriate for sensitive clinical matters.

When a service provider creates, receives, maintains, or transmits PHI on our behalf and qualifies as a HIPAA Business Associate, we address that relationship as required by HIPAA, including through a Business Associate Agreement when applicable.
